Job summary

Rehabilitation Hospital of Oak Creek is a brand-new 40-bed inpatient rehabilitation facility seeking a Licensed Practical Nurse for the night shift. You will assess patients, initiate nursing interventions, and collaborate with an interdisciplinary team to support recovery and discharge planning.

The role requires a Wisconsin or compact LPN license, an Associate's degree, and BLS certification within 30 days of hire.

Qualifications

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire.
• Requires critical thinking, decisive judgement and the ability to work with minimal supervision.
• Experience: One year previous experience preferred.
• Licenses: Active Wisconsin or compact state LPN license required; may need PSOR update before employment.
• Education: Associate's Degree required.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.
