Job summary

Community West Rehabilitation Hospital in Brownsburg, IN is seeking a Licensed Practical Nurse to provide direct patient care on night shifts (36 hours/week, 12-hour shifts, 7:00 PM–7:30 AM). The role works under RN supervision within a rehabilitation-focused team, focusing on comprehensive care plans and timely assessments.

Eligible candidates must hold Indiana LPN licensure, with CPR/BCLS certification; ACLS preferred.

Qualifications

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification. ACLS preferred.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient.
• Performs patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients.
• Under the supervision of the RN, assists with the assessment on all patients and reassessments as per policy.
• Under the supervision of the RN, performs timely and accurate QI assessments.
• Carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness.
