Job summary

Interim HealthCare in Erie, PA is seeking a Licensed Practical Nurse (LPN) to provide in-home patient care with an emphasis on comfort and dignity. You will administer medications, monitor vitals, and collaborate with a clinical team.

The role offers shifts 11pm-7am, daily pay options, up to $34 per hour, and one-on-one patient care in a home setting. A valid LPN/LVN license and CPR/BLS certification are required.

Qualifications

• Graduate of a practical nursing program with active LPN/LVN license in the state
• Current CPR/AED/BLS/First Aid certification required
• Experience in home health care or direct patient care preferred
• Reliable transportation to care sites is required

Responsibilities

• Provide in-home direct patient care per prescribed plan
• Assess and document patient conditions and report changes
• Administer medications, insulin, and IV/fluids as prescribed
• Care for wounds, dressing changes, and personal hygiene support
• Collaborate with providers and care team for optimal outcomes
