Interim HealthCare in Erie, PA is seeking a Licensed Practical Nurse (LPN) to provide in-home direct patient care under the prescribed plan of care. The role emphasizes compassionate support, wound care, and routine assessments to promote patient comfort and dignity.

Candidate must hold an active LPN license in the state, be CPR/BLS/First Aid certified, and have reliable transportation to care sites. Prior trach/vent experience is a plus but not required.

Qualifikationen

• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites and/or work locations.
• Trach/ventilator experience preferred but not required.

• Provide in-home direct patient care according to the plan of care and nursing scope.
• Assess and document patient conditions and report changes as needed.
• Administer medications, insulin and IV fluids with thorough documentation.
• Inspect and care for wounds, change dressings and assist with hygiene.
• Assess for additional injuries or infections and deliver appropriate treatments.
• Collaborate with providers and allied health team to ensure patient outcomes.
