Qualifications

• Active LPN license or in process to schedule/take state exam.
• Graduated from an LPN training program.
• Passion for providing high-quality, person-centered care.
• Strong teamwork and communication skills.

Responsibilities

• Monitor and document residents' vital signs and physical/emotional status.
• Assist with mobility, transfers, and positioning of residents.
• Support with meal service ensuring nutritional needs are met.
• Maintain a clean, safe, and comfortable environment.
• Communicate changes in residents' conditions with nursing staff.
• Assist with medication reminders as directed by providers.
• Contribute to a positive, supportive atmosphere for residents and families.
