Qualifications

• If IV therapy is required, complete an approved IV therapy course or equivalent per state guidelines.
• Attain supervised clinical experience across the life span or 400 hours as an LPN at another facility.
• Unencumbered LPN license with State Board or multi-state licensure; maintain required CEUs and department competencies.

Responsibilities

• Provide nursing care for residents in long-term care under supervision of RN/physician.
• Administer medications and treatments as ordered.
• Document resident interactions and outcomes; maintain confidentiality.
• Communicate clearly with residents, families, and healthcare team.
