Job summary

The Encore at Boca Raton Rehabilitation and Nursing Center is seeking a qualified LPN to join our caring team in Boca Raton, Florida. The position involves monitoring and planning nursing care under the supervision of a Registered Nurse, ensuring continuity of care across shifts, and mentoring CNAs.

Responsibilities include resident and family education and acting as a resident advocate aligned with our mission to provide a high-quality, five-star rehabilitation environment.

Qualifications

• Current/valid Florida LPN license/registration.
• Previous experience in long-term care is preferred.
• Must be in good physical and mental health.
• Ability to work independently or as part of a group.
• Effective communication with others.

Responsibilities

• Monitor, plan, and implement the nursing care of residents within the facility under the direction of a Registered Nurse.
• Ensure continuity of care of residents between shifts by providing direct care as well as supervising the care given by CNAs and support staff members.
• Participate in resident and family teaching.
• Serve as a resident advocate with a focus on the facility’s mission.
