Qualifications

• LPN license required by the state.
• CPR certification required.
• Two years of nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident's interdisciplinary team to provide the best outcomes.
• Identifying and reporting all abnormal changes in residents’ condition.
• Works under direct supervision per state Nurse Practice Act and center policies.
• Delivers nursing care for long-term or rehabilitative needs.
• Collects patient data, makes observations, and reports pertinent information.
• Implements and evaluates the patient plan of care per state regulations.
• Directs and supervises care by other nursing personnel when applicable.
• Maintains knowledge of documentation requirements and equipment use.
