Job summary

RHA Health Services, LLC is seeking a Licensed Practical Nurse (LPN) for the Trenton, TN on-site shift 12 AM to 8 AM. This full-time role supports individuals with intellectual and developmental disabilities and mental health needs, offering meaningful work and a collaborative team environment.

You will administer medications, monitor health conditions, provide health-related training to staff, and work closely with the Interdisciplinary Team to promote safe, person-centered care.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current nursing license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to obtain certification.
• Must communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Assist the Registered Nurse in delivering quality nursing services to persons supported.
• Provide health related training to unlicensed personnel.
• Administer medications and treatments or assist with medication administration.
• Assist with ensuring preventative and routine medical appointments are scheduled.
• Conduct medication closest assessments.
• Collect health related data.
• Observe for and report any abnormal health findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for the people supported and assist with developing community supports to promote optimal health.
