Job summary

Queen City Hospice in Fayetteville, Ohio seeks a compassionate Licensed Practical Nurse (LPN) for night shifts to provide high-quality hospice care. You will deliver patient-centered care focused on comfort, dignity, and quality of life while documenting care and coordinating with the care team.

The ideal candidate holds an active LPN license, CPR, at least 3 years of nursing experience, and a valid driver's license. Night schedule is 3 x 12-hour shifts with every other weekend.

Qualifications

• Graduate of an accredited Practical Nursing (LPN/LVN) program.
• Active LPN/LVN license in the state of practice.
• Minimum 3 years of nursing experience.
• Current CPR certification.
• Strong verbal and written communication skills.
• Excellent organizational and time management abilities.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act.
• Implement nursing care based on comprehensive assessments and individualized care plans.
• Complete accurate and timely clinical documentation.
• Educate patients and families on diagnoses, medications, and plan of care.
• Coordinate hospice services with the interdisciplinary care team.
• Communicate patient condition changes and care needs effectively.
• Deliver pain and symptom management interventions.
• Monitor and evaluate patient responses to treatments and medications.
