Job summary

Queen City Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality hospice care during night shifts. This role emphasizes patient comfort, dignity, and quality of life for patients and their families.

The ideal candidate is clinically skilled, independent, and passionate about end-of-life care. This full-time position features 3 x 12-hour night shifts per week, including every other weekend, at our Fayetteville, Ohio location.

Qualifications

• Graduate of an accredited Practical Nursing (LPN/LVN) program.
• Active LPN/LVN license in the state of practice.
• Minimum 3 years of nursing experience.
• Current CPR certification.
• Strong verbal and written communication skills.
• Excellent organizational and time management abilities.
• Valid driver's license with reliable, insured transportation.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act.
• Implement nursing care based on comprehensive assessments and individualized care plans.
• Complete accurate and timely clinical documentation.
• Educate patients and families on diagnoses, medications, and plan of care.
• Coordinate hospice services with the interdisciplinary care team.
• Communicate patient condition changes and care needs effectively.
• Deliver pain and symptom management interventions.
