Job summary

The Care Team Hospice is seeking two Full-Time Night Shift Licensed Practical Nurses (LPNs) in New Philadelphia, OH 44663 to provide overnight bedside care for hospice patients. Work alongside experienced RNs with a focus on compassionate symptom management and family support.

These positions offer Full-Time hours, night shifts, and a 3–4 nights per week schedule. Join a dedicated interdisciplinary team and contribute to exceptional end-of-life care in the New Philadelphia area.

Qualifications

• Active LPN license in good standing.
• Hospice, home health, long-term care, or skilled nursing experience preferred.
• Strong clinical assessment and communication skills.
• Able to work independently while collaborating with an interdisciplinary team.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Provide skilled nursing care under the direction of the Registered Nurse while following each patient's plan of care.
• Monitor patient conditions, recognize changes in health status, and communicate updates to the care team.
