Job summary

CenterWell Home Health in Illinois is seeking a licensed practical nurse (LPN) for full-time night shifts (7pm–7am) to provide compassionate, patient-focused home health care. You will work under an RN, implement care plans, and support recovery at patients' homes in the Dekalb and Aurora area.

You will educate patients and families on disease management, maintain accurate visit documentation, and coordinate with the care team to ensure timely planning and smooth transitions.

Qualifications

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, and the healthcare team regarding patient needs and status changes.
