Job summary
Apploi is seeking a Licensed Practical Nurse (LPN) for a full-time night shift at our Haven Healthcare-affiliated skilled nursing and long-term care facility in Grayville, Illinois. You will provide direct nursing care, administer medications, monitor residents, and collaborate with RNs and physicians to ensure high-quality patient care.
Requirements include an active Illinois LPN license and graduation from an accredited practical nursing program.
Qualifications
- Active Illinois LPN license required.
- Graduate of an accredited practical nursing program.
- Experience in long-term care, skilled nursing, or rehabilitation preferred.
- Ability to work every other weekend.
- Strong clinical, communication, and organizational skills.
- Compassionate, dependable, and team-oriented.
Responsibilities
- Provide direct nursing care to residents in a skilled nursing / long-term care setting
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to the RN or physician
- Perform wound care and assist with care planning
- Supervise and support Certified Nursing Assistants (CNAs)
- Maintain accurate and timely clinical documentation
- Follow Illinois nursing standards, safety protocols, and infection control procedures
