Job summary

PACS Group, Inc. is seeking an LPN for full-time night shift at Mentor Hills Post Acute. You will direct day-to-day nursing assistant functions, supervise staff, and ensure compliance with policies and procedures.

You will manage medication administration and clinical documentation while coordinating care across departments. Ideal candidates are licensed LPNs with strong leadership and communication skills, committed to high-quality resident care and patient safety, and available for nights

Qualifications

• Must possess a Nursing Degree from an accredited program or equivalent LPN/LVN credential.
• Active license to practice as an LPN/LVN in the state.
• Ability to read, write, speak English and understand medical terminology.

Responsibilities

• Direct day-to-day nursing assistant functions and supervise nursing staff on the unit.
• Ensure nursing procedures follow facility policies and government regulations.
• Maintain accurate charting, medication administration, and treatment plans.
• Plan shifts, services, and activities with staff and physicians.
• Assist in discharge planning and coordination with other departments.
