Oak Hill in Middletown, CT seeks a Licensed Practical Nurse to support community programs. The role is 26 hours weekly with Thursday night through Saturday night schedules, including a $4000 sign-on bonus.

Qualifications

Current Connecticut LPN license in good standing.
Graduate of an accredited Practical Nursing program.
Valid driver's license to transport individuals in agency vehicles.
Current CPR and First Aid certification, or ability to obtain upon hire.
Strong clinical assessment, communication, and documentation skills.
Ability to pass required background checks, fingerprinting, and pre-employment health screenings.
Ability to complete and maintain required certifications, trainings, and regulatory requirements.

Responsibilities

Provide clinical and nursing care to individuals served.
Coordinate medical appointments and follow-up care.
Administer medications, injections, treatments, and other prescribed interventions.
Monitor vital signs and document observations accurately.
Support health needs including G-Tube care and physician-directed interventions.
Maintain medication records and nursing notes.
Ensure compliance with Oak Hill, DDS, DPH and HIPAA regulations.
Assist with orientation and mentoring of new staff.
