Qualifications

• Current and active LPN license in Ohio.
• Minimum 1 year experience in a nursing home.
• BLS certification required.

Responsibilities

• Provide direct patient care by monitoring vital signs and administering medications.
• Assist with daily living activities and care planning under supervision.
• Document observations and changes in patient condition per policies.
• Maintain a safe, clean environment for patients and families.
