Job summary

Eastbrook Healthcare Center is seeking a compassionate and dedicated LPN for the night shift in Cleveland, OH. You will provide direct patient care, monitor vital signs, administer medications, and collaborate with RNs and physicians to ensure high-quality care during nighttime hours.

Qualifications

• Current and active LPN License in Ohio.
• Basic Life Support (BLS) certification is required.
• Minimum of 1 year experience in a Nursing Home.

Duties and Responsibilities:

• Provide direct patient care by monitoring vital signs, administering medications, and assisting with daily living activities.
• Collaborate with registered nurses and medical staff to develop and implement individualized care plans.
• Assist with patient education and promote healthy lifestyle choices.
• Document patient observations, care provided, and changes in patient condition in accordance with facility policies and procedures.
• Maintain a clean, safe, and supportive environment for patients and their families.
• Respond to patient needs and emergencies promptly and effectively.
• Ensure compliance with all health and safety regulations, as well as facility protocols.

Compensation and Schedule:

• Pay: $34/HR
• Full-Time Shifts
• Night Shift (7p-7a) (Rotating Weekends)
• Location: Cleveland, Ohio

Skills and Qualifications:

• Current and active LPN License in Ohio
