Job summary

The Care Team Hospice is seeking two Full-Time Night Shift Licensed Practical Nurses (LPNs) to join our New Philadelphia team. This role offers bedside support for hospice patients during overnight hours, working alongside experienced On-Call RNs.

You will care for patients transitioning, managing overnight symptoms, and supporting families and facility staff. When not urgent, you may perform routine hospice visits, contributing to compassionate, high-quality end-of-life care.

Qualifications

• Active LPN license required.
• Hospice, home health, long-term care, or skilled nursing experience preferred.
• Strong clinical assessment and communication skills.
• Ability to work nights and collaborate with RNs and interdisciplinary team.

Responsibilities

• Provide skilled nursing care under RN direction per patient care plan.
• Monitor patient conditions and communicate updates to the care team.
• Assist with symptom management, medications, treatments, and comfort measures.
• Document care and observations accurately in EMR.
• Collaborate with physicians, RNs, caregivers, and the interdisciplinary team.

This unique role offers the opportunity to provide compassionate bedside support for hospice patients during overnight hours while working alongside our experienced On-Call Registered Nurses.You'll care for patients who are actively transitioning, require overnight symptom management, or need additional support when family caregivers or facility staff need assistance.
