Job summary

The Care Team Hospice is seeking two Full-Time Night Shift Licensed Practical Nurses (LPNs) to join our New Philadelphia team, providing bedside hospice care during overnight hours alongside our On-Call RNs.

As an LPN on the night shift, you will support patients and families, manage overnight symptoms, and participate in routine visits when needed, while collaborating with an interdisciplinary team to ensure compassionate, patient-centered care.

Qualifications

• Active LPN license.
• Hospice/home health/long-term care experience preferred.
• Strong clinical assessment, communication and patient care skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Provide skilled nursing care under the direction of the Registered Nurse while following each patient's individualized plan of care.
• Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team.
• Assist with symptom management, medication administration, treatments, and patient comfort measures.
• Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR).
• Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care.
