Fountains of Franklin is seeking an LPN/Nurse for night shift to provide compassionate, safe care to residents with Alzheimer’s and dementia. You will implement care plans, monitor conditions, administer medications, and document thoroughly while coordinating with families, physicians, and the care team.

Qualifications

• Current LPN/LVN license as required by the state.
• Experience with older adults, memory care, or dementia preferred.
• Strong assessment, communication, organizational, and critical-thinking skills.
• Ability to work independently within a healthcare team.
• CPR/BLS certification or ability to obtain.

Responsabilités

• Provide direct nursing care per care plans and physician orders.
• Monitor residents for changes in physical, mental, behavioral, and emotional condition.
• Administer medications safely and accurately.
• Maintain accurate nursing documentation and reports.
• Collaborate with physicians, families, and staff to adjust care plans.
