Job summary

Salude is seeking a dedicated Licensed Practical Nurse (LPN) for a night shift (7 P.M. – 7 A.M.) in Suwanee, GA. You will deliver quality nursing care, participate in discharge planning, and coordinate with an interdisciplinary team to ensure resident-centered outcomes.

Qualifications

• Must hold and maintain a current Licensed Practical Nurse (LPN) license.
• Must hold and maintain a current CPR certification within 90 days of hire.
• License in good standing with the state; no findings of abuse, neglect, or misappropriation of resident property.

Responsibilities

• Deliver quality nursing care using the nursing process with an outcomes focus.
• Identify patient needs and contribute to discharge planning and follow-up during stay.
• Develop individualized care plans in collaboration with residents and care team.
• Implement care plans consistently, focusing on resident-centered outcomes while using available resources.
• Collaborate with physicians and other health professionals by sharing updates to the care plan.
• Provide and maintain optimal resident care and comfort with current nursing practices.
