Job summary
AccordCare is seeking a Licensed Practical Nurse (LPN) to deliver compassionate, one-on-one skilled nursing care in the home setting, promoting client health, independence and dignity.
This full-time, on-site role features a 11:00pm–11:00am schedule, starting Sunday, Monday, and Thursday, with weekly pay, paid orientation, and benefits after one year. Ideal candidates are Georgia-licensed, CPR-certified, and experienced with EMR documentation and home-health care.
Qualifications
- Licensed as an LPN and in good standing with the Georgia Board of Nursing.
- 1+ year clinical experience preferred.
- CPR certification.
- Experience with EMR documentation.
- Able to work independently in home health.
- Compassionate and client-focused; strong communicator.
- Experience with G-tube, trach, or vent care.
- Driver's license with reliable transportation.
Responsibilities
- Provide skilled nursing care tailored to each client's needs.
- Monitor health conditions and document changes in clinical status.
- Administer medications and perform wound care.
- Deliver G-tube, trach, or vent support when required.
- Educate and guide clients and families to support ongoing health at home.
- Communicate with the care team to ensure continuity of care.
