Qualifications

Associate degree or state nursing certificate required.
One year of nursing experience with the served population.
Current LPN/LVN licensure in state of residence.
Valid driver’s license, car registration, and auto insurance.
CPR/FA, HIV/AIDS, BBP, and ACT certifications where required by state licensing.

Responsibilities

Monitor health conditions and ensure quality services.
Administer medications and document; supervise medication staff.
Provide physician-ordered treatments and personal care.
Conduct ISP-driven health care assessments & respond to condition changes.
Train staff on precautions, infection control, and medication administration.
Coordinate with other providers and external agencies to ensure consistent care.
Provide training on self-medication, prescribed treatments, sex education, and smoking cessation.
