Oxley Park Health & Rehab is seeking a dedicated LPN to provide high-quality, patient-centered care in a supportive, mission-driven setting. You will work with RNs, CNAs, and the interdisciplinary team to administer medications, monitor residents, and document care plans.

Responsibilities
- Administer medications and treatments as prescribed.
- Monitor residents’ conditions and report changes to the care team.
- Provide direct nursing care in accordance with care plans.
- Supervise and support CNAs in delivering quality care.
- Maintain accurate and timely documentation.
- Ensure compliance with all clinical policies and regulatory standards.
