Job summary

City Scape Rehabilitation and Care Center is seeking compassionate LPNs for full-time night shifts in a 60-bed skilled nursing environment in Colorado. The role focuses on direct resident care, supervision of nursing assistants, and adherence to nursing standards and regulations.

Qualifications

• Currently licensed as an LPN in Colorado.
• Prior experience in a nursing community.
• Ability to read, write, speak, and understand English.
• Ability to work flexible hours.

Responsibilities

• Assist routine daily direction and monitor the work of the nursing assistants, ensuring high nursing care standards.
• Communicate regularly with the Nurse Manager or Director of Nursing about observations and concerns.
• Document changes in patient conditions including nutritional issues, infections, or weight changes.
• Participate in shift reports to ensure proper information transfer about residents.
• Document and notify departments of patient room changes, LOA, new admits, or discharges.
• Assist in admission, orientation, and observation of new residents and ensure orders are communicated.
