The role emphasizes collaboration with the healthcare team, accurate documentation, and adherence to safety protocols, with wage negotiation and a potential extra $5,000 opportunity. Benefits are included.

Qualifications

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.
• Background checks as required by state regulations.

Responsibilities

• Provide direct nursing care and treatments within your scope of practice.
• Collaborate with RNs, physicians, and interdisciplinary team members.
• Monitor resident conditions and communicate changes using the SBAR process.
• Participate in resident assessments, evaluations, and individualized care planning.
• Administer medications and treatments safely and accurately.
• Document resident care, observations, and interventions according to guidelines.
• Review physician orders and communicate with healthcare providers.
• Respond during emergencies and perform CPR when necessary.
• Assist with admissions, discharges, and transitions of care.
• Support medication management initiatives.
