Job summary

Signature HealthCARE is seeking a caring LPN for a full-time night shift position in Tennessee. You will provide direct nursing care, supervise aides, and coordinate resident care plans within a supportive team.

Qualifications

• Current, active LPN/LVN license in Tennessee.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• CPR/BLS certification preferred.
• Experience in a supervisory or charge nurse role is preferred.
• Strong clinical observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the care team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by Nursing Director.
