Henry County Health and Rehabilitation in Paris, Tennessee is seeking an LPN for night shift (7 PM – 7 AM). You will supervise CNAs, participate in admissions and transfers, assess residents, and administer medications per orders to deliver compassionate resident care.

The role emphasizes care planning, communication with supervisors, and maintaining patient health information confidentiality in a team‑oriented environment.

Qualifications

• Active Tennessee LPN license.
• Current CPR/BLS certification.

Responsibilities

• Direct the daily activities of CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
