Lake Behavioral Hospital in Waukegan, IL is seeking an Intake Admission Clinician to manage referrals, assess patients of all ages, and support discharge planning under the Director of Admissions and Referrals. You will be the primary community contact and collaborate with physicians for care decisions.

Requirements include an RN license or a master's degree in a related field, with 2+ years in intake assessment in a psychiatric setting. Illinois licensure preferred.

Formación

• Registered Nurse or Master's degree in a human relations field required.
• Two or more years of intake assessment experience in an acute psychiatric setting preferred.
• Illinois licensure as a mental health professional preferred.
• Knowledge of DSM and Mental Status Exam and addiction/recovery field.

Responsabilidades

• Document and process inquiry calls per Intake policies.
• Conduct patient triage for appropriate placement or refer to community resources.
• Assess and stabilize patients before discharge or coordinate admission arrangements.
• Notify and assist on-call doctor with patient interviews.
• Obtain administrative approval for all admissions.
• Complete admission paperwork per hospital policy.
• Maintain an intake log with required information.
• Make follow-up calls to no-show patients.
• Provide guest relations and train staff as needed.
