Job summary

Lake Behavioral Hospital is seeking a dedicated PRN Intake & Assessment Clinician to manage referrals, assess individuals of all ages, and collaborate with physicians for optimal care decisions at our Waukegan, IL facility.

The role requires a Master’s degree in a human relations field and IL licensure as a mental health professional. Two years of intake assessment experience in an acute psychiatric setting is preferred, with knowledge of DSM, Mental Status Exam, and Addiction & Recovery.

Qualifications

• Master's degree in a human relations field is required.
• Licensed to practice in Illinois as a mental health professional (Social Worker, Counselor, or RN with MHP).
• Knowledge of the latest DSM and Mental Status Exam; understanding of Addiction and Recovery field.

Responsibilities

• Manage referrals and service requests and coordinate with physicians for care decisions.
• Conduct patient triage for placement or referral to community resources.
• Assist with discharge planning and admissions paperwork following hospital policy.
