Responsabilidades

• Assess patient preferences and barriers to involvement in care, including values and cultural needs.
• Develops, implements, and documents plans of care with goals in collaboration with the interprofessional team and family.
• Promotes respect and empathy in care delivery across diverse and vulnerable populations.
• Evaluates changes in patient condition and updates the care plan in EMR with the team.
