BJC Home Care offers patients and their families a full range of home care services, including hospice and supportive care. The role involves night shifts (7:00 PM–7:30 AM), full-time 36 hours, at Evelyn's House, with weekend/holiday rotations and benefits eligibility.

Prior hospice experience is preferred and drivers license is required. Responsibilities include comprehensive patient assessment, care planning, and interprofessional collaboration to ensure high-quality, patient-centered care

Qualifications

• Nursing Diploma or Associate's degree is required.
• RN licensure is required.
• Valid driver's license is required.
• 2–5 years of nursing/home health experience is preferred.

Responsabilités

• Assess patient preferences, barriers, and needs across physical, emotional, spiritual, and cultural aspects.
• Develop, implement, and document individualized care plans with defined goals in collaboration with the care team and family.
• Promote equity and empathy in care delivery and advocate for patient-centered continuity of care.
• Evaluate changes in patient condition and adjust care plans in collaboration with the interprofessional team and caregivers.
• Work in a safety-sensitive role requiring alertness at all times.
