Job summary

Ohio’s Hospice in Dayton, OH is seeking a Hospice Crisis Care LPN for a full-time night shift. The LPN will drive to patient homes within the Dayton/Springfield area and provide 1:1 bedside care under RN supervision.

The role requires an active Ohio LPN license, CPR certification, and the ability to document care using a computer. Driving in various conditions and background checks are required.

Qualifications

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen

Responsibilities

• As a Hospice Crisis Care LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.
• This role will provide 1:1 bedside care for patients that are actively declining.
