Transitions Hospice seeks a compassionate Certified Nursing Assistant to cover overnight shifts as a Companion Aide CNA. You will maintain bedside presence, monitor safety, and assist with personal care to support patients and families during the long night hours.

The role emphasizes calm observation, rapid reporting of changes to the nurse, and diligent adherence to care standards in a hospice setting at our Rockford location.

Competenze

• Current CNA certification meeting state training requirements.
• Current CPR certification.
• 1+ year of hospice, home health, or related healthcare experience preferred.
• Prior experience in overnight or night shift patient care preferred.
• Ability to work independently and remain alert during overnight hours.
• Strong observational skills to recognize and report changes.
• Genuine compassion for patients and families navigating end-of-life care.

Mansioni

• Overnight bedside presence and patient supervision to ensure safety and comfort.
• Monitor patient condition and environment; report significant changes to the on-call nurse.
• Prevent falls, elopement, or self-harm within CNA scope of practice.
• Respond promptly to patient requests and maintain a calm overnight environment.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
