• Utilize screening tools to identify levels of care
• Develop patient-centered treatment plans to improve health and wellness
• Deliver ongoing evidence-based therapeutic interventions for patient and family
• Coordinate care with other facilities, providers, and insurers to ensure seamless services
• Maintain effective communication with medical staff, case managers, and families to facilitate care
• Attend care conferences and interdisciplinary rounds when needed
• Document clinical notes and recommendations for insurance reviewers and other agencies
• Engage in process/ group/family therapy as part of caseload
• Ensure timely documentation for billing and care coordination
