Job summary

Qualifications

• Master's degree in social work, counseling, or a related field is required.
• One to three years of related experience is required
• BLS certification required
• Crisis intervention skills, cultural competence, integrity, and courteous disposition required.

Responsibilities

• Provides assessment, crisis intervention, and referral services.
• Coordinates admission and treatment with psychiatry and other physicians.
• Documents intake information in the electronic medical record and Doc to Doc.
• Liaises with hospitals, community resources and regulators to transition patients to ongoing care.
• Demonstrates strong telephone etiquette and teamwork within a care team.
• Operates clinical software (bed board, Tele-tracking, web x-change, EPIC, Outlook).
