Job summary

Qualifications

• Strong interpersonal and teamwork skills.

Responsibilities

• Monitor and evaluate patient conditions, update care plans, and coordinate care.
• Participate in performance improvement initiatives and maintain professional standards.
• Communicate care plans with patients, families, and the care team.
• Support nursing leadership and mentor other staff as needed.
