Job summary

UNC Health Nash is seeking a dedicated Registered Nurse to provide competent clinical nursing care in a home health setting within Nash Counties, NC. Reporting to the Nurse Manager/Director, you will coordinate patient care using the nursing process while delivering quality care in a home health environment.

This role requires strong clinical skills, effective communication, and collaboration with an interdisciplinary team to meet patient needs and ensure a high standard of care.

Qualifications

• Graduation from a school of professional nursing.
• Licensed to practice as a Registered Nurse in the state of North Carolina.
• One year of nursing experience caring for acute and chronically ill patients or an equivalent combination of education and experience.

Responsibilities

• Education—Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs.
• Evaluation of care—Identifies expected patient outcomes and collaborates with the interdisciplinary team.
• Implementation—Provides competent care for patients with complex problems according to nursing standards and procedures.
• Leadership—Participates in goal setting and professional development within the unit.
• Patient Assessment—Assesses and synthesizes data to plan and communicate care.
• Planning—Collaborates with patient and family to update the plan of care.
• Research—Keeps abreast of journals and brings ideas to the team.
