Job summary

Responsibilities

• Assesses patient clinical and psychosocial needs and creates care plans.
• Partners with patients/families to implement and evaluate care.
• Ensures safe patient transitions and coordinates with interdisciplinary team.
• Educates patients and families and documents education provided.
• Completes all required clinical and quality documentation per RHS standards.
• Delegates and prioritizes tasks to maintain quality patient care and respond to emergencies.
