Qualifications

Responsibilities

• Demonstrates ability to assess patient clinical and psychosocial needs and applies standards of practice in the Critical Care/Telemetry units.
• Partners with patients and families to develop, implement and evaluate plan of care.
• Assesses and incorporates evidence-based standards of practice.
• Collaborates with interdisciplinary team in care coordination.
• Facilitates safe patient transitions to the next care setting as part of the plan of care.
• Demonstrates urgency to facilitate patient throughput and ensure quality care.
• Educates patients and families, coordinates discharge education, and documents understanding.
• Accurately completes required clinical and quality documentation per RHS standards.
• Ability to delegate and prioritize tasks to ensure patient care needs are met.
