Job summary

Baptist Trinity Hospice, part of the LHC Group and affiliated with Optum, is seeking a CNA to provide personal care and support to patients in their homes. The role includes assisting with daily living activities, monitoring health, and ensuring safety in a home care setting.

The position requires a valid driver’s license and reliable transportation, with state-specific training requirements to be met within the first months of employment.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation
• TN: Successful completion of a total of seventy-five (75) hours of training including sixteen (16) hours of clinical training prior to or during the first three (3) months of employment and ability to provide basic services, including simple procedures as extension of therapy services, personal care and household services essential to health care at home.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
