Job summary

Qualifications

• Graduated from an ACPE accredited College of Pharmacy (required).
• Post-Graduate Year 1 (PGY-1) Pharmacy Residency or three years of relevant work experience (preferred).
• Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky (required).
• Current board certification by Board of Pharmacy Specialties (preferred).
• Current Basic Life Support and ACLS as determined by leadership (required).

Responsibilities

• Reviews medication orders for accuracy, appropriateness and efficacy.
• Documents all pharmacy interventions in patient EMR and CDS.
• Provides pharmacokinetic consultations and dosing recommendations as required.
• Participates in formulary management and cost avoidance documentation.
• Mentors pharmacy technicians and students; supports residency/scholarship activities.
• Covers shifts and assists in policy development under pharmacy leadership.
