Kaiser Permanente seeks an experienced Registered Nurse to coordinate admissions and post- hospital care planning in collaboration with physicians and families. The role emphasizes safe discharge planning, cost-effective care, and compliance with regulatory standards across inpatient and post-acute settings.

You will engage with a multidisciplinary team, manage complex care transitions, and ensure timely, high-quality patient outcomes in a fast-paced hospital environment.

Competenze

• Two years RN experience in acute care setting or case management.
• California RN license and Basic Life Support.
• Ability to apply utilization review, discharge planning and case management principles.
• Knowledge of regulatory/accreditation requirements (TJC, Medicare, Medi-Cal, etc.).

Mansioni

• Plans, develops, assesses and evaluates care for members.
• Collaborates with physicians and multidisciplinary team to develop care plans.
• Recommends alternative levels of care and ensures compliance with requirements.
• Assesses high-risk patients for post-hospital planning.
• Develops and coordinates discharge plan and communicates to involved parties.
• Monitors hospital stay and ensures timely, efficient service delivery.
• Coordinates interdisciplinary care planning and authorizations for outside services.
• Conducts daily clinical reviews for utilization/quality management.
• Acts as liaison between in-patient and referral facilities for case management.
• Refers patients to community resources for post-hospital needs.
• Coordinates patient transfers and maintains required documentation.
• Adheres to regulatory requirements and accreditation guidelines.
• Educates team members and patients to support discharge and coping with illness.
• Reports incidents and participates in utilization monitoring and data analysis.
• Participates in rounds, conferences, and various work projects as assigned.
