Advocate Health is seeking Advanced Practice Clinicians to join inpatient hospitalist teams across Illinois. Positions include Christ Hospitalists with night shifts, 4 on 4 off, and weekend options, with cross coverage and admits in a hybrid work setting.

Qualifications

• Demonstrated high level of clinical proficiency, and excellent decision-making skills.
• Ability to work independently and as part of a health care team.

Responsibilities

• Evaluate, diagnose, treat, and manage acute, critical and chronic illnesses in inpatient settings.
• Take histories, perform physicals, determine differential diagnoses and prescribe treatments.
• Order and interpret diagnostic tests and coordinate care across teams.
• Educate patients on health, disease, and self-care strategies.
• Provide care coordination, discharge planning, and referrals.
• Prescribe pharmacologic and non-pharmacologic therapies across settings.
• Prepare care plans and maintain accurate medical records.
• Provide clinical direction to nursing staff and ensure patient care quality.
• Adapt care plans for age-specific needs throughout life span.
