Job summary

SunriseVista Behavioral Hospital is seeking a Registered Nurse (RN) Intake Assessor in Canton, OH. You will perform initial psychiatric assessments, evaluate crisis situations, and decide on urgency of admission while coordinating with psychiatrists and care teams.

The role emphasizes risk assessment, de-escalation, and payer authorization processes, with a Day Shift 7a–7p schedule and pay ranging from $28 to $35 per hour.

Qualifications

• RN licensed to practice by state board.
• 2+ years in acute behavioral health or crisis evaluation preferred.
• Strong clinical assessment, risk assessment and de-escalation skills.

Responsibilities

• Conduct comprehensive mental health evaluations of incoming patients.
• Evaluate patients in acute psychiatric crisis and determine risk.
• Implement immediate safety measures when needed and decide admission urgency.
• Determine inpatient vs higher level of care and coordinate with team.
• Communicate with social workers, EDs, law enforcement, and families.

How You Will Make a Difference

Initial Psychiatric Assessment

• Conduct comprehensive mental health evaluations on incoming patients
• Evaluate patients in acute psychiatric crisis
• Determine level of risk (e.g., suicidal ideation, psychosis, aggression)
• Implement immediate safety measures if needed
• Decide urgency of admission or need for higher level care

Admission & Level-of-Care Decisions

• Determine if the patient meets criteria for inpatient psychiatric admission
• Recommend appropriate placement:
• Inpatient psych unit
• Collaborate with psychiatrists or providers for final decisions

Coordination & Communication

• Communicate with:
• Social workers and case managers
• Emergency departments, law enforcement, or family members
• Gather collateral information to support assessment

What We're Looking For

• Education: Associate's Degree in Nursing (ADN) from an accredited program. (BSN preferred but not required.)
• License: Active, unencumbered RN license to practice by the State Board of Nursing.
• Experience: 2+ years acute behavioral health or crisis evaluation experience preferred. New graduate RNs with strong clinical assessment skills are welcome to apply.
• Other: Expertise in risk assessment, de-escalation, and payer authorization processes. Must be 21 or older.
