Qualifications

• Must be at least 21 years old.
• Valid driver's license and acceptable driving record.
• 1+ year of relevant work experience.
• Knowledge and skills in English usage, interpersonal relationships and excellent communication skills.
• Ability to implement and interpret programs, goals, objectives, policies, and procedures.
• Demonstrated ability to hold strong personal boundaries and build rapport with patients.
• Geographic area knowledge to ensure effective routing.
• Ability to organize and prioritize competing priorities.
• Competency in patient rights, safety and sanitation.

Responsibilities

• Interact routinely with patients and communicate significant observations to clinical staff.
• Follow reporting and communication guidelines with staff and patients.
• Complete daily logs and forms accurately.
• Maintain daily schedules and structure around transport, meals, transitions and programming hours.
• Provide patient observation rounds and supervise activities during shifts.
• Promote recreational outings and pro-social skills with patients.
• Ensure wellbeing and maintain a positive, structured environment for patients.
• Assist with transport of patients to and from facilities.
• Document information regarding client care per procedures.
• Utilize de-escalation skills to maintain safety for patients and staff.
