Job summary

Accurate Home Care in Forest Lake, MN seeks a Personal Care Assistant (PCA)/CFSS Support Worker to provide direct patient care and assist with daily living activities under supervision. This is a part-time role with evening/night shifts on Friday/Saturday, and occasional Monday coverage.

Qualifications

• Interpersonal skills and compassion for clients.
• Fluent English language required.
• Must be at least 16 years of age and able to drive.
• Valid driver1s license and reliable vehicle are required.
• Ability to lift up to 50 lbs.
• Certification: PCA/CFSS where available.

Responsibilities

• Provide direct patient cares according to the patient care plan.
• Assist patients with daily living activities (bathing, feeding, grooming, dressing).
• Communicate with Clinical Manager and scheduling team.
• Perform light housework and activities supporting independent living.
• Report changes or concerns to Clinical Manager.
