Stonecreek, located in Paducah, KY, is seeking a Night shift LPN/RN to join our 90-bed facility. The role centers on coordinating patient care, administering medications, and collaborating with a healthcare team to ensure high-quality, compassionate service for residents.

The ideal candidate will hold a current LPN/RN license in Kentucky with at least one year of LTC experience preferred and familiarity with MatrixCare.

Qualifications

• Current LPN/RN license in State.
• At least one year of LPN or RN experience in a Long‑term Care Setting is preferred.
• Knowledge of MatrixCare preferred.

Responsabilités

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications.
• Records patients medical information and vital signs
• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays
• Initiates corrective action whenever the patient displays adverse symptomatology
• Provides bedside care for a wide variety of medical patients
• Changes dressings, inserts catheters, and starts IVs
• Prepares equipment and aids the physician during the examination and treatment of the patient
• Responds to life‑saving situations based upon nursing standards and protocol
• Participates in discharge planning
• Utilizes time management skills
• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms
• Performs other position‑related duties as assigned, depending on assignment setting
