Job summary

Cleveland Clinic Lutheran Hospital in Cleveland, OH is seeking a Practical Nurse (LPN) to support the 4D Medical/Surgical and Telemetry Unit. You will assist nursing staff, administer medications, and participate in care planning while enhancing your skills through hospital-wide education resources. The position requires an Ohio-registered LPN, BLS, and experience in patient care.

Night shifts are from 11:00 p.m. to 7:30 a.m.

Qualifications

• Graduate of an approved Practical Nursing program.
• One year of clinical experience within the previous three years or graduate within the previous six months of hire.
• LPN licensure and current Ohio registration required.
• Certification in medication administration and IV therapy as approved by Ohio Board of Nursing for specific areas.
• BLS certification within one year of hire.

Responsibilities

• Perform accurate admissions, ongoing assessments and discharges within defined time frames; document pain levels.
• Collaborate with RN on care plans and notify RN of pertinent findings.
• Provide direct patient care and complete hourly rounding and logs.
• Delegate and evaluate care by non-licensed personnel.
• Administer prescribed medications and treatments per licensure and protocols.
• Apply knowledge of medication indications and side effects; follow 5 rights of medication administration.
• Ensure medications are secured at all times; report errors or near misses.
• Assess educational needs of patient and family; provide teaching on diagnosis, tests and procedures.
• Review discharge instructions with patient after physician/RN presentation.
• Complete annual competency requirements and training.
